Cardiovascular protection has become an increasingly important consideration in the management of metabolic and cardiovascular disease. Her systematic review and meta-analysis evaluates the cardiovascular effects of glucagon-like peptide-1 receptor agonists (GLP-1 RAs), examining evidence from major cardiovascular outcome trials and comparing their effects with SGLT2 inhibitors.
Key Research Highlights
The analysis included nine completed cardiovascular outcome trials involving 77,684 randomized participants. The findings demonstrated a significant reduction in three-point major adverse cardiovascular events (MACE) among patients receiving GLP-1 receptor agonists compared with placebo.
The pooled analysis reported a 15% reduction in three-point MACE, while additional evidence indicated reductions in cardiovascular death, non-fatal stroke, heart failure hospitalization, all-cause mortality, and composite kidney outcomes.
An important finding was the variation in heart-failure outcomes according to baseline heart-failure status. The analysis identified a significant benefit for the composite of heart-failure hospitalization or cardiovascular death among patients without a previous history of heart failure, while the effect was not statistically significant among those with baseline heart failure.
The research also compared GLP-1 receptor agonists with SGLT2 inhibitors through a network meta-analysis involving 14 trials and 117,633 patients. While both drug classes demonstrated comparable effects on MACE reduction, SGLT2 inhibitors showed greater benefits for heart-failure hospitalization and renal outcomes.
