🩺 Clinical Context

Accurate early prognostication after out-of-hospital cardiac arrest (OOHCA) remains difficult. Scoring systems such as MIRACLE2 and NULL-PLEASE have been introduced to assist clinicians, but direct comparative evidence between these tools is limited.

🎯 Research Purpose

This study sought to evaluate and compare the ability of the MIRACLE2 and NULL-PLEASE scores to predict survival and neurological outcomes following OOHCA.

🔬 Study Approach

A retrospective, single-centre study was conducted involving 39 consecutive patients who achieved return of spontaneous circulation after OOHCA. Demographic, clinical, and outcome data were obtained from electronic health records, and both prognostic scores were calculated for each patient.

Primary endpoint: Survival to hospital discharge

Secondary endpoint: Favourable neurological outcome (CPC ≤ 2)

Predictive performance was assessed using ROC curve analysis (AUC) and Mann–Whitney testing.

📈 Outcome Summary

67% of patients survived to discharge

63% achieved favourable neurological recovery

🔹 MIRACLE2 Score:

Demonstrated limited prognostic value for both survival and neurological outcomes.

🔹 NULL-PLEASE Score:

Showed strong discrimination for survival and meaningful prediction of poor neurological recovery. Higher scores were associated with increased mortality and adverse neurological outcomes.

⚖️ Comparative Assessment

Overall, the NULL-PLEASE score outperformed MIRACLE2, offering superior early risk stratification in patients following OOHCA.

🧠 Practice Implications

The NULL-PLEASE score may serve as a more reliable tool to support clinical decision-making during post-resuscitation care.
